Health System Electronic Nursing Documentation Optimization
Burden associated with nursing documentation is well documented in the literature. A needs assessment at the project site identified concerns with the usability and time nursing documentation took from direct patient care. Vendor supplied timestamp data verified the project site was at the 50th percentile ranking. The redesign of the nursing reassessment section of the electronic nursing flowsheet was performed to decrease duplication, redundancy, and non-value-added tasks. The redesign standardized nursing documentation to decrease burden. The results included an 11% decrease in time in minutes in hyperspace at the project site; decrease of 88-97% time in minutes in flowsheets; decrease of 88-97% in number of steps needed to complete reassessment documentation; and time savings of 1.5-6.5 minutes per patient per reassessment. The project demonstrated improving the usability and functionality workflows through this evidence-based change decreased documentation time, redundancy, and click burden resulting in improved productivity. The revisions elevated the project site to the 25th percentile ranking amongst like institutions in the vendor database.
