In response to the COVID-19 pandemic and to protect the safety of athletes and volunteers, a nonprofit organization serving over 40,000 registered athletes with intellectual and developmental disabilities canceled all sports physical events in 2020. The nonprofit organization had to determine the necessary changes to operational processes and safety protocols to safely resume the sports physical events. Once these are established and implemented, the sports physical events can resume. The original purpose of this project was to create an addendum to the sports physical. Once the addendum was piloted, volunteers would evaluate these practice changes, and the addendum would be either edited, used for statewide events, or discontinued based on volunteer feedback. Continual changes in guidance related to COVID-19 created concern that an addendum would be nonapplicable before being implemented, so the addendum was not completed, and the organization postponed the event until 2022. The project’s purpose pivoted, and the team used a fault tree analysis, a top-down approach to determine the root cause of failing to establish an addendum to the sports event toolkit.en_US
